AMPHIBIAN STUDY. With a $2.1 million grant from the National Institutes of Health, researchers at Yale and Pennsylvania State University will investigate the causes of deformities in amphibians. The study will focus on four states: Connecticut and Pennsylvania, which have lower than average rates of deformities, and New York and Vermont, which show higher than average incidences. The researchers are interested in whether the problems may be caused by trematodes, a group of parasites known to cause fatal diseases in humans. Trematodes must be carried by snails as hosts before they can affect amphibians, and, according to Yale ecology professor David Skelly, who was among the researchers awarded the grant, snails may be proliferating. "People may be doing things to wetlands that inadvertently turn wetlands into snail farms--such as making wetlands deep, introducing nutrients, or cutting trees to let in more light, " he said. BROOK RESTORATION. Connecticut Department of Environmental Protection fisheries biologist Brian Murphy is managing an effort to restore the natural flow of Railroad Brook in Bolton; the stream's original path was destroyed by railroad construction. The restoration involves using a backhoe to excavate a new channel through nearby wetlands. The new channel is lined with rocks that range in size from gravel to boulder; the rocks are intended to provide habitat for aquatic plants and animals. Strategically placed logs and boulders should create bank undercuts, bends, and curves that will, if necessary, dissipate the brook's energy and prevent floods. The design should allow deep pools to form, in which the stunted native brook trout and black-nosed dace that now populate the stream can grow to normal size. WHAT A GAS. A new state-of-the-art system for managing the 65-acre landfill near the Manchester-East Hartford border offers major environmental benefits, according to town officials. The system consists of a series of 30 collecting wells, which feed the gas generated by the landfill into a network of pipes; eventually, the gas is burned off. The system also collects leachate, which is pumped to a separate system and sent to a wastewater treatment plant. This reduces the amount of liquid that leaves the landfill. The landfill stopped accepting garbage in January, 2000; the town now sends its waste to the Hartford-based Connecticut Resources Recovery Authority for incineration. INVASION OF THE PHRAGMITES. The state Department of Environmental Protection DEP ; is collaborating with the Nature Conservancy in an experimental program to eliminate phragmites, a reed grass that is choking out many other native species along the Connecticut River. The project, at Lord's Cove, a rare brackish tidal marsh, is an emergency measure to stem the invasive plant, and requires the unusual step of spraying the area with the non-selective herbicide Rodeo. Since the state prohibits aerial spraying of pesticides, the work must be done from the ground, using amphibious vehicles to spray the pesticide in 50-foot swaths. Lord's Cove provides habitat for several bird species, including the bald eagle. Currently, phragmites grow on 200 of the cove's 1, 100 acres; the plant is spreading at a rate of 30 acres a year and desloratadine.

Of all musculoskeletal injuries, sprains and strains are the most common, with ankle sprains alone reported to account for 15 percent of all sports injuries Almekinders 1999, Liu and Nguyen 1999, McGriff-Lee 2003, Rubin and Sallis 1996 ; . While such injuries are usually considered minor, they should be taken seriously due to their potential to cause chronic pain and functional instability Almekinders 1999, Osborne and Rizzo 2003 ; . Once the decision is made to try sacral nerve stimulation, Denson will do a trial run with the patient using an external stimulation device. Under local anesthesia, a set of wires are placed under the skin near the sacral nerves. The wires are connected to an external stimulation device. If the patient responds to stimulation, a second procedure is scheduled and a permanent stimulator is implanted under the skin and attached to the wires previously implanted. If, however, the patient does not respond to the test stimulation, the wires can be easily removed. InterStim was developed by the biotechnology firm Medtronic, using research done by Emile Tanagho and Richard Schmidt of the University of California School of Medicine in San Francisco. The initial trials occurred in 1988 and 1989, and the FDA approved the first device for clinical use in 1997. Despite this long history, it is still almost unknown outside of the urologic medical community, and only a handful of Texas physicians are trained in its use, Only about six hundred physicians worldwide are trained in the technique. Why this lag in acceptance? According to Medtronic spokesperson Carol Barnett, it is not unusual for a medical device to take as long as fifteen to twenty years to become widely accepted in medical practice. During that time the device maker and independent researchers do extensive testing in a wide variety of applications to determine if the device will continue to work with long-term use and whether any unanticipated side effects turn up. Also, physicians must be trained to recognize patients who might benefit from the procedure, how to implant the device and how to properly calibrate the device for the best results, all of which take time. You start with a handful of trained physicians, says Barnett, and those physicians bring along a few other physicians and, after many years, you have enough physicians trained in the technique to have widespread availability of the procedure. According to Medtronic's research, seventyone percent of those who had experienced several episodes per day of heavy bladder leakage had at least a fifty percent reduction in the number of leaks when studied at the end of the first year. Stipulate truthful disclosure of the indications, contraindications, and side effects. But in a cultural environment steeped in the Ideal Model, the government's regulation of pharmaceutical advertising faces daunting epistemological challenges. From the standpoint of business ethics, false or misleading advertising is universally regarded as immoral and illegal. For pharmaceutical products, however, "truth in advertising" is hampered by the nature of the information, which is cloaked in arcane scientific language and adorned with impenetrable and often misleading or erroneous statistical data. In the absence of transparency, pharmaceutical "truth" has become socially constructed by physicians, pharmacists, and even lawyers and juries who are legally empowered to serve as the translators and interpreters of this otherwise inscrutable, if not unreliable, information. Moreover, consumers of pharmaceutical products have also been impeded by the kind of information that the FDA requires pharmaceutical researchers to ferret out in clinical studies. FDA guidelines require testing for safety and effectiveness relative to placebos, but they do not mandate testing relative to well-established OTC drugs and prescription drugs already on the market. This institutionally enforced blind spot has created a pharmaceutical market crowded with expensive "me too" drugs, while physicians have no FDA-validated scientific basis for prescribing an inexpensive older drug whose patent has expired ; instead of an expensive, patent-protected new drug. Indeed, it is highly likely that many, if not most, of the "me too" drugs on the market today are less safe and less effective than the older drugs, but for the most part no one really knows. To advertise truthfully that a new drug is safer or more effective than an old drug, drug companies would have to conduct expensive head-to-head clinical studies, which might not turn out in their favor. Moreover, thanks to the industry-friendly regulatory processes, old drugs that have been proven safe and effective are less profitable than new drugs. From the industry's standpoint, why waste money mass marketing old drugs whose profitability has been undermined by competition from inexpensive generic alternatives? In a market plagued by artificial monopoly, imperfect information, and conflict of interest, we now have a system in which reliable old drugs are less likely to be advertised and less likely to be prescribed than patent-protected new drugs whose safety and effectiveness remain shrouded in mystery. Under the Ideal Model of Health Care, normal risk taking is distorted by the illusion of scientific objectivity. No other industry is legally required to divulge so much unreliable, useless, and contradictory information in its advertising in order to protect the public from so many remote or unproven harms. After all, television advertisements for sports utility vehicles are not required to divulge potential rollovers; cell phone advertisements are not required to divulge potential auto accidents; and fast-food chains are not required to divulge potential obesity from consumption of their products. The problem here is that under our current physician-centered system, the most powerful stakeholders--physicians, pharmacists, and trial lawyers--benefit from a zero-risk standard of drug safety. Again, it is important to note that the FDA has not clearly demonstrated any measurable degree of competence in regulating drug research, drug labeling, or drug and mebeverine.
By Lexie Stark as told to Debbie Stark Note from Debbie: We've all been on a diet or two at some time in our lives--some successful, others not. Many of our Goldens have to watch their weight, too, and whether you're a human or a dog, it can be a tough process. Lexie, an 83-pound red Golden rescued in 2000 GRR# 00-012 ; , is here to tell you all about it. She's 12 years old, and very spry for her age and size. When my forever family adopted me, I was a little heftier than I should have been. I think the papers said 96 pounds! I'm sure that must have been a clerical error--although, considering my love for good food OK, any and all food ; , I guess I might have put on just a few pounds here and there. But I certainly didn't think any of those "overweight" words applied to me! . words like "heavy, " "chunky, " "fat, " and that awful "obese" --that one stays with you for a long time. Over the past few years, I've been on many diets I guess you could compare them to the fad diets so many people try ; , and although I'd lose a bit of weight, I was always able to get someone to sneak me a treat. But too many treats pretty much stop any and all weight loss. So one day, Mom decided it was time to diet for real. Looking back, I think I can pinpoint when everything changed for me: the treats started coming out of the refrigerator instead of the cabinet, and they were called "baby carrots." I came to a frightening conclusion: "Oh my God, this time she's really serious." Other changes soon followed. More vegetables were added to my bowl in an attempt to cover the fact that there was less food--did she think I wouldn't notice? I knew exactly how many pieces of kibble I got at each meal. How was I to keep count now? Did a green bean count the same as a kibble? More important, could, for instance, cilodan opthalmic. LOCALIZED CHRYSIASIS, A DISFIGURING SKIN CONDITION INDUCED BY LASER THERAPY. A. V. Klinkhoff, A. B. Arthur, A. Chalmers, J. K. Rivers, Mary Pack Arthritis Centre A 20-year old woman developed seropositive nodular RA in 1980. She was treated with standard disease modifying regimens over a 20 year period, which included two courses of gold. Her first course was discontinued in 1986 after 5, 020 mg. Her second course, between 1997 and 1998 amounted to 3, 325 mg, and was discontinued for loss of effect. At present, her RA is controlled with a combination of methotrexate and D2E7 in the setting of a clinical trial. In January 2001, she underwent laser hair removal using Qswitched Nd: YAG laser. This immediately induced a dense blue-grey skin discoloration of the cheek in the treated area. This has remained stable during 9 months of follow-up. Chrysiasis is a well known complication of long term gold therapy which typically occurs on sun-exposed skin of individuals who have received more than 8 grams of gold. There is one previous report of localized chrysiasis induced by laser therapy Arch Dermatol 1995 Dec; 131: 1411-4 ; . Localized chrysiasis can be induced by short-pulsed Q-switched lasers in patients who are currently receiving gold or, more importantly, in those who have received gold in the past. This form of chrysiasis is caused by a structural alteration in dermal gold deposits, and appears to be irreversible. As laser dermatologic treatments are now common, rheumatologists should inform their patients about this rare but cosmetically disfiguring complication and combivir.

All studies on multiepisode schizophrenia that examined the effects of treatment with typical antipsychotics revealed a significant increase in basal ganglia volumes, in particular nucleus caudatus, and decreased grey matter volume in different cortical regions Table 2 ; , predominantly in frontal grey matter volume. These volume changes were detectable even after a 12-week treatment. In contrast to these results, treatment with atypical antipsychotics do not seem to change basal ganglia volumes in previously neuroleptic-naive patients. Moreover, switching from typical to atypical antipsychotic treatment reduces the increased basal ganglia volume to normal values compared with healthy controls. Only the volumes of thalamus and cortical grey matter increased after atypical antipsychotic treatment Table 2 ; . Brain structural changes over time in schizophrenia are in general a matter of debate [26]. Although there is growing evidence for grey matter loss and ventricular enlargement in studies of patients with first episode schizophrenia, the extent and the nature of these changes are vividly debated. The mechanism of this remarkable different effect of typical and atypical antipsychotics remains unclear. The volume changes detected by MRI may simply be due to changes in tissue perfusion, fat or water content [26]. Other mechanisms have, however, been discussed to explain the increase in basal ganglia volume by typical antipsychotics. Owing to their high density of dopamine D2 receptors, basal ganglia are a major target of dopaminergic pathways. The strong antagonistic effect on D2 receptors of typical antipsychotics may chronically lead to a proliferation of these receptors resulting in an increased metabolism, blood flow and size of these structures [27]. This hypothesis is in accordance with findings in rat brains that were exposed to typical antipsychotics. In these studies enlargement of striatal structures was observed as well [28, 29]. Atypical antipsychotics also have antagonistic effects on D2 receptors, but they have smaller affinity and occupancy on this receptor than typical antipsychotic agents. The observed changes in brain structure may, however, reflect progressive neuropathological changes suggested in at least a subform of schizophrenia [30]. According to this, atypical antipsychotics may slow down or stop this process, whereas typical antipsychotics may not. Via agonism on N-methyl-D-aspartate receptors [31, 32], increased expression of neurotrophic factors [33, 34] and stimulating of neurogenesis [35, 36] atypical antipsychotics may improve the pathophysiology and the neuronal degeneration process in schizophrenia. Up to now there are no published post-mortem studies in schizophrenic patients treated with atypical antipsychotics and zidovudine and ciloxan, for example, ciloxan ointment.
