The objective of this study was to evaluate the relation of dietary intake of carbohydrates with incidence of coronary heart disease CHD ; in middle-aged men living in eastern Finland. Methods The subjects were 1981 Finnish men 42 60 years of age examined in 1984 1989 in the prospective Kuopio Ischaemic Heart Disease Risk Factor Study. Dietary intakes were assessed by a 4-day food-record at baseline. Cox proportional hazards models were used to analyze the association between dietary intake of carbohydrates and incidence of acute coronary events in men free of CHD at baseline. Smoking, family history of CHD, diabetes, systolic and diastolic blood pressure, body mass index, maximal oxygen uptake in exercise test, serum LDL and HDL cholesterol concentration, and intakes of alcohol, saturated fat and total energy were considered as confounders. These factors were eliminated from the full model unless their joint removal resulted in a change of 10 % in the relative risks RR ; of the main exposure. Age and examination year were forced in the models. Results During an average of 12.3 years of follow-up, 249 acute coronary events were documented. Dietary intake of carbohydrates measured in grams per day was inversely associated with the incidence of acute coronary events.
